Lake County Medicaid/CHP+ Programs

Lake County’s Medicaid/CHP+ programs provide pregnant women and children under the age of 18 with free or low cost health insurance. Focusing on insuring that Lake County’s children are able to receive the necessary health care is a primary objective. 

The Lake County Medicaid/CHP+ Programs Provide the Following:

• Medicaid/CHP+ applications                                                                

• Counseling

• English/Spanish translation

• Community outreach program held

the 2^nd Wednesday of every month 

from 11:00 AM – 2:00 PM at the

meeting room of the Post Office

• Assistance in completing Medicaid/CHP+ forms

• Pregnancy Tests
