Lake County Public Health and Human Services to Co-Locate Services and Offices in Collaboration with Care Clinic

This October, Lake County Public Health and Lake County Human Services are moving and co-locating to 735 Highway 24, Leadville, CO 80461.
The 8,400 square foot facility will be utilized by the County as a new consolidated home for Lake County Public and Environmental Health, Lake County Human Services, and ancillary services including new primary and behavioral care services provided by Summit Community Care Clinic (Care Clinic).
The location will be familiar to many in the community, as it was formerly the location for Dr. Lisa Zwerdlinger’s Rocky Mountain Family Practice, which was subsequently leased by St Vincent Family Health, which vacated that facility earlier this month. Lake County Commissioner Sarah Mudge states,
“Consolidating public assistance programs into one facility will reduce barriers to care by providing more continuity, including broader awareness of the range of services we offer under one roof.”
Human Services will continue to offer the following services at the new location starting late October. Please call 719-486-2088 for more information:

- TANF (Temporary Assistance for Needy Families)
- Child Support
Public Health will continue to offer the following services at the new location starting October 5th. Public Health accepts Medicaid, Medicare, Private Insurance, and offers a free or sliding fee scale for the uninsured. Please call 719-486-8181 to schedule an appointment:

- Harm Reduction - Naloxone and Drug Testing Strips

Plus! In a new partnership with the Care Clinic, the location will also offer new primary and behavioral health services starting September 29th. Patients who wish to schedule a Primary Care or Behavioral Health visit with the Care Clinic in Leadville can contact 970-668-4040. The Care Clinic accepts Medicaid, Medicare, Private Insurance, and offers a sliding fee scale for the uninsured.
